Do the items on the FIHS pertain to the child/adolescent or the parent?

eg. Item 7 problems related to release from prison the child/adolescent or the parent?

Eg. Problems related to unwanted pregnancy that the child /adolescent was an unwanted pregnancy or that they currently are carrying an unwanted pregnancy.

If you could please provide some clarification around this and what was envisaged we would be most grateful.

In general , unless otherwise stated the FIHS items pertain to the child/adolescent. So for the examples you've given (unwanted pregnancy), this should be used to describe the situation of the child /adolescent.

Note that the current set up of the FIHS allows scope for recording situations where the circumstances of the child/adolescent's parents or carers introduce special complications to treatment. For example, where a parent has been imprisoned this could be recorded as a factor under 'Problems related to primary support group'.

As a general point, it's useful to note that the FIHS is a very rudimentary measure that requires further development. We introduced it as part of the MH-CASC study in response to advice from child & adolescent specialists who argued that a whole range of social circumstances act as complicating factors in the provision of services and need to be taken into account in understanding why some consumers receive more care than others. The argument was that these factors are not adequately reflected in the clinical diagnosis and should be recorded separately. As no simple scale was available for routine use, we turned to the ICD10 coding for 'factors influencing health status' as a first generation attempt to begin gathering relevant data. The result is a simple 7 item checklist. There's a few grad research theses waiting here for anybody wanting to refine the checklist and produce a more sophisticated scale.
